Before the incident
At approximately [REDACTED] on [REDACTED] [REDACTED] a maintenance employee was working on a magnetically coupled cylinder on the mini-bag [REDACTED] system. During the trouble shooting the injured employee asked an assisting employee to apply “strong” force on the cylinder to help in repairing it.
What happened
The employee pushed and the pry bar the injured employee was using slipped allowing the mechanism to move further and more forcefully than anticipated. The cylinder impacted the employee’s left pinky fracturing the distal phalange and lacerating the tip of the pinky.
