Using a buffing wheel and buffing a part called ALPHA

On [REDACTED] [REDACTED] (Finisher) was polishing the top of a part named Alpha (image attached). There is a 2 1 4 cutout on the part and that cutout made contact with the cotton medium at approximately [REDACTED] the wheel grabbed the part and pulled [REDACTED] toward the machine. [REDACTED] 's right hand was holding the part with his right ring finger inside the cutout holes and when the wheel pulled the part his finger was still in the cutout resulting in a fracture to his right ring finger. After the incident [REDACTED] continued to work inside the shop and at [REDACTED] left the grind and polish room. At approximately [REDACTED] he contacted [REDACTED] (Weekend Project Weld Supervisor). At Approximately [REDACTED] [REDACTED] reached [REDACTED] EHS. [REDACTED] sent [REDACTED] to Urgent Care in [REDACTED]. [REDACTED] was not scheduled to work on [REDACTED]
