What happened
Employee was working in the vicinity of a conveyor system and heard a loud noise coming from the equipment. Getting closer to investigate his break away lanyard became caught between the rollers and the belt. When attempting to pull the lanyard free his fingers became pinched between the belt and roller. Once pulled away he noted a laceration and a fracture to the pointing and index finger of his left hand.
