Before the incident
Pushing a cage when cage door fell on employee
What happened
On [REDACTED] around [REDACTED] employee approached me stating that the upper door of the shelf cage fell on to his shoulder. He expressed that he was pushing a rack into the staging area when the cage door fell onto him. I asked him if he wanted to go to the hospital and he refused medical attention. In addition I looked at his injured shoulder and saw no abnormalities or bruising. I told him to take a break and ice his shoulder. When he came back he continued on working. However after an hour passed he told me his shoulder was hurting and that he wanted to seek medical attention so I took his statement and brought him to Renown Regional Medical Center ER.
