What happened
employee set the middle shelf on a split APC up with all locks in place. He proceeded to attempt to pull the bottom retaining gate out to enclose the bottom portion of the APC. While pulling on the gate the middle shelf came dislodged from the safety latches and struck him on the top of his head. He drove himself to the emergency room where he received 8 staples on his head.
