What happened
Employee slipped and fell while unloading the 10 [REDACTED] truck from the plant by pulling 2 unloaded pallet jacks out toward the dock. He fell approximately 3 to 5 feet from the door striking both the pallet jack and the floor directly. His phone came out of his pocket and he had skin marks on his knees despite wearing jeans. His foot is swollen and may have a fracture.
