on [REDACTED] [REDACTED] reported to supervisor had told him he was having pains in his arm and was calling an ambulance when we arrived out there he was in his vehicle llv we did ask if still needed an ambulance and as to what was going on he stated no and he had pains in his back. He went to hospital and later returned with a knee brace and on crutches
