Carrier 's Statement 2 carriers involved. Employee 1 - I was on my way back to [REDACTED] around [REDACTED] when a lady in a tan minivan with a white sticker on the side of the van was backing out of a driveway. I felt I had to slam on the brakes and honk. I stopped in time and did not collide with the minivan. Bue before I knew it another Employee slammed into the back of my truck. I got out of my truck to see of everyone was okay before returning to the office to report the accident. the accident occurred on [REDACTED] believe the address [REDACTED] close to the office. this accident occurred on [REDACTED] around [REDACTED]. Employee 2 injured - A lady was packing down a downward driveway quickly that was on a downward incline on [REDACTED] as I noticed her doing these action I proceeded to break my vehicle instantly which caused me to run into the back of Employee 1 's vehicle which cause the accident. My neck snapped back which I have pain currently.
