A Forklift driver did not notify him by blowing the horn and employee fell over the raised forks and hit his head. [REDACTED] employee was backing up near the [REDACTED] hut as forklift was coming around the corner. Employee backed into the forks and fell. There is a mirror located at this intersection that if used would have allowed each employee to notice the other person at the intersection.
