Employee approached to the operation manager [REDACTED] and asked if the Concentra Clinic called and reported about his hospital visit on [REDACTED] [REDACTED] Since the employee is still on modified duty assignment due to preexisting injury from [REDACTED] the manager [REDACTED] and I assumed that was related to existing injury and then [REDACTED] contacted [REDACTED] from HR office to discuss Later then the employee was talking to [REDACTED] from HR that he went to Concentra Urgent Care on [REDACTED] for the chest pain which he claimed that was due to work related stress The doctor from Concentra Urgent Care suggested him to open a new worker comp file as it was different case from the preexisting injury
