Employee was working alone operating payloader unhooking chains off “130-inch” flasks.

The chains became stuck as they “slacked” inside of the flasks. Employee raised the bucket on the pay loader to remove the slack from the chains. While physically moving one of the chains off the flask one of the other chains with hook dislodged from the flask and “slammed” into employee’s right knee.
