What happened
Employee opened the door to the pattern packer so the sensor could stop the machine but did not manually turn off the switch that was located above the sensor door to stop the bottom conveyor which left it running. Employee proceeded to get in between the two pattern packers bent down and as he was reaching to get the box loose under the pattern packer so production could keep on going his glove on the first right hand index finger (thumb) got caught by the rotating chain conveyor pulling his thumb in and causing an amputation at tip of the thumb to the lunula area.
