Before the incident

Replacing cam lock fittings and hoses on the Loading rack.

What happened

Employees were tasked with changing the cam lock fittings and hoses on the R and S loading rack. They worked the West side of the rack and replaced the hoses on the working platform above the truck loading rack. This work was done using fall protection due to operations leaving the grating lifted up on the work platform. This work was completed by [REDACTED] as at truck arrived to be loaded. On [REDACTED] employees returned to complete replacing the fittings and hoses on the East side of the loading rack. Once the retrieved the fittings that were needed they replaced one hose on the East side. Once completed the employees began to move to other hoses in the area to change. For unknown reason the injured employee went over to the west side of the loading rack and accessed the working platform and fell through the open grating that was left up by operations.
