What happened
A [REDACTED] was assigned to replace two sets of oil filters on a gas turbine. The employee had already completed replacing filters on one set without incident. This task used a 1 1 8 socket and impact with a one and one-eighth wrench. The employee started on the other oil filter casing and he realized that the bolts were different sizes and needed the proper 1 one-fourth socket to use the impact for the bolt. He proceeded to use two one-fourth inch wrenches to remove the bolts and as he tried to remove the last bolt the wrench slipped off striking the employee in the upper lip.
