What happened

A Turner millwright crew were permitted to install a seal on a pump. During the job walk they noticed the scaffold needed to do work on was not yet completed. The two employees went back to the maintenance shop to take a break. After one employee had finished his break he walked to the north side of the shop to see if the client millwright was in the [REDACTED] room to see if the job was ready. After realizing he was not there he proceeded to walk back to the south side of the shop to head toward the control room. While walking in that direction the employee slipped on a plastic flange covering that was on the ground causing him to fall to the ground. Employee used his left hand to brace his fall and his hand contacted a pallet causing a fracture to his pinky finger.
