The evening of [REDACTED] supervisor on duty. stated that he needed to go home due to his back hurting. I asked him did he hurt his back at work and he stated that he thought he hurt his back the previous day on [REDACTED]. I asked did he report it to anyone and he stated no he didn 't tell anyone on [REDACTED] but he told the morning supervisor the [REDACTED]. I informed him that was failure to report a accident. He stated that he would just go to the chiropractor the following day on [REDACTED] to have his back cracked.
