What happened
On [REDACTED] at approximately [REDACTED] [REDACTED] [REDACTED] on states she was working in the High-Speed Tray Sorter HSTS operation placing a tray inside a box when [REDACTED] who was operating a Riding Pallet [REDACTED] tried to squeeze into the pallet to pick up the box. As he tried to lift the box the pallet [REDACTED] went faster than expected and hit the pallet with the box on top of it. The pallet lift hit my right ankle causing an injury. [REDACTED] states that she blew her horn but [REDACTED] didnt hear it because she had on headphones. She states that employee backed into the pallet that she was picking up with the pallet [REDACTED].
