On [REDACTED] I went to employee to address her unscheduled absence from [REDACTED]. She told me that she had to call out because her back was hurting. I asked if she was okay and she said yes I believe so; no further conversation about her hurt back was made throughout the day. On [REDACTED] she notified management that her back was still hurting and that she told Supervisor on [REDACTED] that she had an on the job injury from [REDACTED]. I had her come to see me in which I asked her at what point during our conversation on [REDACTED] did she mention an on the job injury; to her response was no I just mentioned that my back was hurting and did not get into the details. After our conversation we sent her to Concentra to get evaluated.
