What happened

To whom it may concern: this is in reference to a accident on [REDACTED] that Employee was involved in located at [REDACTED]. I received at [REDACTED] stating that she had just fell and injured herself. I immediately tried to get in touch with my station manager to tell him what had happened and also to let him know that I was by myself and what he wanted for me to do He instructed me to go out to the scene to investigate the issue in which I did! After arriving at the scene I noticed that Employee was sitting in the LLV. I ask her how she was feeling Her response was that her right ankle was hurting. By this time I contacted cca and another Employee to come and help deliver what was left on [REDACTED] and to also deliver the one split she had on [REDACTED]. Then I continued to question Employee as to what happened She said that after delivering a package to [REDACTED] as she was walking back to her vehicle that she tripped and fell on a water meter that was sticking up about four inch
