employee was in the ladies room in the last stall. She states she noticed water on the floor when she sat down to use the bathroom and when she got up she grabbed the safety bar and then proceeded to unlock the stall door and when she went to walk out of the stall she slipped and fell on her right knee. She then hopped out of the ladies room and saw [REDACTED] and asked where AMDO [REDACTED] was. she then saw union rep and he took her into the union office until AMDO and SDO arrived in union office to investigate the accident.
