The employee tripped twice on the safety floor mats on [REDACTED] on the semi. Two mats with a gap between them were on the floor. She pivoted to put oversized mail in the APC to her right. The first time she tripped; she stepped between the two mats and caught her foot the rubber connections to connect to another mat. She caught herself on the rail and twisted her back. The second time she tripped; she caught her foot under the torn mat. Again she caught herself on the rail and twisted her back. She twisted her back on the lower center right side of the back hip and right leg. She was unable to lift the right leg. Another employee helped her into her car.
