What happened
On [REDACTED] at about [REDACTED] I picked up a heavy box. As I put the box into the APC it was supposed to go in some weights fell out of the box. I proceeded to put them back inside the box and taped it shut. After doing this my back began to hurt. As the day went on the pain got worse to the point I couldn 't work [REDACTED]. I tried to work the following day but the pain became increasingly worse to severe pain.
