When: [REDACTED] How: claims she tripped over straps that were piled on floor that was closer to door 8 then door 9. Then somehow fell at an angle hurting her left side of her head and hip. she could not pull herself up into a chair had to be help due to her pain. there is no witness only after the fact. Root cause being in attention could not interview employee she went in ambulance for medical care. She told everyone not to touch her until management arrived. Was given Ice pack for head and hip.
