Before the incident
The fusion line had an issue where it caused a “fold over” while it was exiting the oven and the employees had to create slack in the flooring to be able to pull the product backwards on the line. Slack was created towards the end of the line and the flooring needed to be pulled backwards (in reverse) to fix the “fold over.”
What happened
After creating slack in the line in an effort to fix the fold over [REDACTED] had grabbed the side of the flooring product and attempted to pull the product backwards through the the rollers. While pulling the flooring he felt a sharp pain in his right bicep. [REDACTED] had over-exerted his right bicep while pulling the flooring. [REDACTED] mentioned he cannot lift his arm above his head from the pain currently.
