What happened
The forklift driver was unloading a truck containing packaging. He had unloaded two pallets that were located at the very tail end of the truck. In order to unload the last two pallets the forklift needed to enter the truck. As the forklift back tires rolled over the dock plate the truck began to roll forward which caused the forklift to fall from the receiving dock onto the pavement.
