Before the incident
The employee was using a manhole hook to slide a cement communication vault back so that he could visually inspect what was inside.
What happened
After opening the vault about 10-12 inches he put down his hook and went to look inside. He could not see what he needed to see so proceeded to pick up the lid with his bare hands. It slid away from him causing the lid to fall from about 6-7 and crush his right hand.
