What happened
The 2nd shift closed die press crew had loosened the clamp nuts to the upper 2133 block die on the 40KTP. Subsequently loosened clamps were then pointed perpendicular to the die clamp channels in order to clear and lift the press free. The IW reached back in to readjust a clamp and suffers finger fractures and lacerations as the press rises.
