Before the incident
The employee was making “W” coils on the Pedrick Coiler when he noticed an alignment issue causing an unwanted bend in the coil. This prompted the [REDACTED] to re-align the coiler clamp and die assembly.
What happened
The employee was using a set-up bar placed between the coiler clamp and die assembly when he placed his right fingers between the coiler clamp and the die while checking the alignment adjustment he had made. The clamp inadvertently closed pinching his right-hand ring and middle fingers between the clamp and the die.
Injury or illness
Open fracture of the right-hand ring and middle fingers. Once at the emergency room the tips of the right-hand ring and middle fingers were removed just before the first knuckle.
