EMPLOYEE WAS ADJUSTING THE DIE TO FREE A PART BECAUSE THE PART WAS OBSTRUCTED AND WOULD NOT ALLOW THE CUT PARTS TO MOVE. INSTEAD OF SEEKING HELP OR USING A PROPER INSTRUMENT HE USED HIS HAND TO REMOVE THE PART. WHEN THE PART CAME LOOSE HIS HAD SHOT UPWARDS HITTING VERY SHARP TEETH ON THE MACHINE
