What happened
Colleague was working on line #3 in Thermo. While observing the machine the colleague noticed their counterpart attempting to remove the scrap but it became stuck and suddenly maneuvered to assist in removing the scrap. While attempting to move the colleague moved sideways with their right foot making contact with the ergonomic mat causing them to roll their ankle in turn their reaction in attempt to catch themselves was to step over with their left foot which caused the mat to move and ultimately leading to them falling onto the floor with their right arm planted on the floor attempting to break their fall out to the right side of the body.
