What happened

On [REDACTED] at approximately [REDACTED] colleague and a forklift operator were removing wooden crates from the new racking system. The pallet was stuck mid-way in so therefore the forklift forks couldn’t reach. The colleague went into the racking system to push the skid up some so the forklift operator can reach for the forklift. The colleague gave the forklift operator the ok to grab and lift the skid. The operator didn’t see the colleagues right hand was still on the pallet so therefore the oeprator procceded to lift the skid resulting in the colleagues right pinky being smashed between the racking and skid. Following the incident it was immediately reported with the colleague being moved to the [REDACTED] room for treatment.their finger was wrapped and immediately referred to St. Mary’s ER for treatment where he received 14 stitches. Following investigation it was found that the skids placed in the rack were improperly placed and normal removal practice was not followed. Col
