Before the incident
Removing bolts on a raw water piping valve (2C104MOV2) utilizing an impact wrench.
What happened
Two employees were removing the [REDACTED] valve at [REDACTED]. Since access to the studs bolts was limited due to piping configuration and a catwalk that had recently been installed the employees had to use a swivel with the socket and impact wrench to remove some of the studs bolts. At the time on the injury one employee was holding the socket (with swivel) in place in place while another employee was operating the impact wrench from the opposite side. When the impact wrench was activated the swivel shifted towards the socket and pinched the tip of the employee’s ring finger.
