[REDACTED] was sitting in WC at nurses station I was coming out of the medication room I saw [REDACTED] stand He then got off balance and attempted to sit back onto the wheelchair When he sat back he sit on on the front edge of the seat and the wheel chair started to roll back and was coming out from under him I ran across the room grabbed him under the arms and pulled him back into the chair At this point his legs were straight out infront of him and he had slid down on his back but no part of his body was touching the floor I pulled him from this position back into the chair to prevent him from injuring himself left shoulder CK
