What happened

The employee was lining up a triplex pump to remove oil from a frac tank on [REDACTED]. During this process the field operator stepped up on the skid and over the swab water line to verify that the swabs were receiving water. While attempting to step back over the swab water return line to leave the area the field operator’s right foot caught the bolt running across the top of the return line causing the field operator to lose balance and fall approximately 12-inches to a lower level. The employee was evaluated at the contracted offsite occupational medical clinic on the day of the injury and was returned to work for regular duty. The employee had follow-up appointments on [REDACTED] and [REDACTED] and was released to RTW regular duty with an MRI scheduled on [REDACTED].
