Before the incident
Operator was servicing molds in a knelt position from the custom platform. Using a custom 'T ' tool to remove insert holder from the mold itself the employee lost his balance and felt a pain in lower back.
What happened
According to the employee as he was turning the insert holder from the knelt position he lost his balance and nearly fell off the platform but managed to grab the safety chain. In doing so he sprained his lower back. He reported to the supervisor that his back has been bothering him for a while. The supervisor not believing the injury was work related offered to move the operator to another oven that wouldn 't require kneeling and he agreed. After working on the 220-3 oven for another 3-4 hours he got his right arm stuck in a tank and asked for assistance from the lead and the supervisor on removing his arm but made no complaints about his back. Frustrated the operator decided to punch out and wait in the break room for his ride home. (note: this claim is currently being contested but is being reported due to unsettled contentions). No witnesses or evidence could be provided in the investigation to support the injury occurred in the manner the employee described.
Injury or illness
The operator was treated for lumbar sprain and right forearm contusion with abrasion. He was placed on 5lb restrictions.
Object or substance involved
It is proposed to have been the result of ergonomics no specific tool or action could be identified for the operators back issue. The forearm injury was directly caused from the metal sensor ring that was molded into the tank the operator got his arm stuck in.
