What happened
Employee said he was unloading top tooling from punch rail on Accupress explained he had loosened the tool but couldn 't get it to move so he loosened die rail further. Then noticed it was being stopped by different source. He loosened source of block but didn 't tighten rail slide he had over loosened properly. When he loaded next tool set in (very heavy) he slid it off cart onto punch rail and began to slide it across to position. When die reached the overly loosened punch rail(section 2 of 3) it fell directly down out of slide and hitting the employee 's finger.
