The coil being run on the 400 Ton press walked over the one coil keeper arm that was installed on the inside of the coil (removable adjustable keepers are installed on both the inside and outside of the coil since the uncoiler does not have a permanent back plate). The employee climbed up behind the mandrel (between the coil and the large stationary tail-out guard) and attempted to place the outer laps of the coil back onto the coil itself.

When the out laps of the coil were moved the sensor no longer saw steel so it signaled the uncoiler to rotate. The employee did not have time to react and was struck by one of the coil keepers that were installed on the outside of the coil. The employee’s leg was pinned between the rotating coil keeper and the tail-out guard.

Contusions and abrasions to the employee 's inner left thigh and groin area.
