What happened
Employee was investigating a mechanical issue. During investigation of the mechanical issue the employee was using a 6 feet A-frame ladder to reach a flour gate in the ceiling of the old air compressor room next to the flour blower. Employee was wearing slip resistant shoes. While trying to reach the gate the employee stepped onto the top rung of the ladder and lost his balance. The employee did not have anybody supporting or securing the ladder. This caused the employee to fall landing on his left foot. The employee broke his leg just above his left ankle. The employee could not be moved due to pain. An ambulance was called to transport the employee to the ER.
