Before the incident
Putting plate clamp rigging devices (dual chain slings attached to a lifting eye with plate clamps at the end of both slings) on a storage rack with the assistance of another employee
What happened
The plate clamp rigging device fell off the storage rack because both clamps were on one side instead of one draped over the other side. The injured worker attempted to catch the clamps and the lifting eye attached to the slings came down and hit the worker on the top of the head.
