What happened
Turned on the pallet jack to take pallet to the lift gate there was a steep incline and the lift gate was wet. Brake was engaged but the pallet jack slipped due to wet surface. Employee attempted to stop the pallet jack from sliding off the truck and pallet jack ran over left foot.
