Making 1 4 inch cut on wood material.
What happened
Employee had to make a 1 4 on some material. As he was pushing the material the 1 4 piece being cut went between the blade and the throat plate and into the table saw and his index and middle finger of his right hand followed and got cut by the blade. There were no guards on the table saw and a push stick was not used.
