Before the incident
Employee was working with the forklift operator to remove the bottom die of the press brake machine.
What happened
Employee instructed the forklift operator to lift one end of the die with the bolts on the side of the die. Once the die was raised the press operator placed a board with his left hand under the die during the lifting the bolts slipped off the fork causing the die to slam down on employees hand.
