Employee prompted the client to change into pajamas and start their bedtime routine.

The client escalated and began to attack the employee. Client hit employee with palms and hands and bit into employee 's right forearm. Client hit employee with a 30-oz. metal cup for several minutes. Employee was beat by the cup on the back of her head neck back. Client then bit employee 's right forearm creating puncture wounds.

Bruises on neck back right shoulder blade left shoulder. Twisted tendon in right index finger twisted left wrist two or more ribs out to the right bite on right forearm with scabbing scab and carpet burn on left knee.

Client 's hands and teeth. Metal cup that client used to beat employee.

Employee was attached by client resulting in multiple injuries. Employee was beat with a metal cup and suffered injuries to the back of their head neck and back. Client also bit employee on right forearm.
