He was using a forklift to retrieve a steel chute in the chute storage area
What happened
He needed to rotate the chute 180 degrees for proper installation. He lifted the chute off the rack with the forks then backed up into a clear area. He raised the forks to waist height. The forks were spread far apart. He got off the forklift to manually spin the chute on the forks. As he was spinning the chute it fell between the forks and landed on the top of his left foot
