Employee was operating a structural mill when material being fed through the mill became jammed causing the operator to stop the line.

The employee placed their right hand on the material to help guide it back through the punch and began jogging the mill in reverse to free the jam. The employees right index middle and ring fingers were positioned such that they were inside a knockout in the jammed material. When the material began backing through the physical guarding around the punch their fingers became pinched between the guarding and the knockout in the material. As the material continued to advance backward the edge of the knockout cut into through the employees fingers.
