Stepped into an unknown liquid on the floor says he was not looking down (so he did not notice it). He slipped and fell.
Injury or illness
Associate initially reported that his right leg slid forward causing him to fall. He initially reported pain on right side middle of back and soreness. Left shoulder also. On the morning of 12 18 24 associate reported that pain in his left shoulder was still evident. Upon hearing this report associate was sent to the occupational health clinic for evaluation.
