Before the incident

What happened
Injured worker and witness was troubleshooting propane forklift in maintenance shop area. Injured worker had adjusted the propane line leading from supply tank to evaporator and was cycling the ignition switch on the forklift where the operator would normally start stop the ignition. During one of the cycles the forklift ignited a present propane fumes that had accumulated during the troubleshooting process. The ignition caused a fume deflagration which engulfed the injured worker and caused burns to their face neck and both arms.
