What happened
The employee was attempting to access the catalyst box on top of the cooler by using the top surface of the 4 hydraulic oil filters next to the engine as a transition point to the adjacent piping to gain access. He had a catalyst element in his hand during this transition. As he was climbing up to the catalyst box he initially stepped onto the 4 hydraulic oil filter cluster then started to step up to the piping with his right foot his left foot that was still on the filters slipped causing him to fall landing on his left side on both the filter cluster and surrounding piping.
