replacing a hydraulic hose on forklift

The production chief was replacing a hydraulic hose from the lower part of the [REDACTED]. As he was securing the new hose he stuck his left arm between the cross bar and the lower part of the mast (cross bar was 10 to 12 high). The cross bar fell down on his left arm below the elbow pinning and trapping his arm. He called for help and two of the production team members came to his aid. They used long pry bars to lift the cross bar free his arm. [REDACTED] was called and he was taken to the emergency room for evaluation
