Before the incident
Employee was on-call and came in for a CT scan. She was administering test injection of saline w the power injector and it was noted that the IV hub was leaking. IV connection was tightened and re-accessed. When she was removing the tegaderm the saline and blood was mixed underneath and it sprayed on employee.
